Feeding Your Cat Through FIP Treatment: A Nutrition Protocol

Cats with FIP are frequently anorexic at the start of treatment. The combination of persistent fever, systemic inflammation, nausea from the disease process, and the stress of illness suppresses appetite in most cats with active FIP. At the same time, FIP causes significant muscle wasting and protein depletion — the cat's body is consuming its own muscle mass to meet energy and protein demands that food intake is not covering.

Nutrition during the 84-day GS-441524 protocol is not a secondary consideration. It directly affects the cat's ability to tolerate treatment, maintain immune function, rebuild muscle mass, and recover body condition. A cat that is eating well during treatment recovers faster and more completely than a cat that is not. Why Nutrition Matters During FIP Treatment

Cats are obligate carnivores with high protein requirements. Unlike dogs or humans, cats cannot reduce their protein catabolism rate when dietary protein is insufficient — they continue to break down muscle protein regardless of intake. A cat with FIP that is not eating is losing muscle mass every day, which compounds the weakness and immune suppression caused by the disease itself.

GS-441524 suppresses viral replication, which resolves the fever and systemic inflammation that are suppressing appetite. Most cats show returning appetite within 3 to 10 days of starting treatment at the correct dose. But the first 3 to 10 days — and the weeks of recovery that follow — require active nutritional management to prevent further depletion and support the recovery process.

The nutritional goals during FIP treatment are:

  • Maintain caloric intake to prevent further muscle wasting
  • Provide adequate protein to support immune function and muscle rebuilding

What to Feed: The Core Principles

High-Quality Animal Protein Is the Priority

The most important nutritional requirement for a cat with FIP is adequate high-quality animal protein. Choose foods where the primary protein source is a named animal protein — chicken, turkey, fish, rabbit — listed as the first ingredient. Avoid foods where the primary protein source is plant-based or listed as generic "meat by-products" without specification.

Protein requirements for a recovering cat are higher than for a healthy cat. The muscle mass lost during FIP illness is rebuilt from dietary protein. A cat that is eating a high-protein diet during treatment will rebuild body condition faster than a cat on a lower-protein diet.

Wet Food Over Dry Food During Active Treatment

Wet food — commercially prepared, heat-processed canned or pouch food — is strongly preferred during active FIP treatment for several reasons:

  • Higher moisture content supports hydration and kidney function, particularly important in cats with wet FIP who have lost fluid and protein
  • Stronger aroma is more appealing to cats with reduced appetite — warming wet food to just below body temperature (around 35 to 37°C) intensifies the aroma further
  • Easier to eat for cats that are weak or have reduced jaw strength from muscle wasting
  • Easier to syringe-feed if the cat is not eating voluntarily — wet food can be thinned with warm water or low-sodium chicken broth to a consistency suitable for syringe feeding

If the cat was on dry food before FIP and refuses wet food, soften dry food with warm water or low-sodium broth. The goal is caloric intake — the best food is the food the cat will eat.

Fresh Protein Sources as Appetite Stimulants

Plain steamed or boiled chicken breast, turkey, or white fish — with no seasoning, no onion, no garlic, no salt — can be used to stimulate appetite in cats that are refusing commercial food. The strong natural aroma of fresh protein is often more appealing than commercial food to a cat with reduced appetite. Shred or flake the protein into small pieces to make it easy to eat.

Fresh protein sources are appropriate as appetite stimulants and appetite supplements, not as the sole diet. They do not provide the complete nutritional profile — vitamins, minerals, taurine — that a cat requires. Use them alongside a complete commercial diet, not as a replacement.

Feeding Frequency and Portion Size

Offer small amounts frequently rather than large meals. A cat with reduced appetite and gastrointestinal involvement from FIP is more likely to eat a small portion than to face a full bowl. Offer 4 to 6 small meals per day rather than 2 large ones. Remove uneaten food after 20 to 30 minutes and offer fresh food at the next meal — stale food is less aromatic and less appealing.

Managing Appetite Loss in the First Week

The first 3 to 7 days of treatment are the most challenging nutritionally. The cat is still febrile, still systemically unwell, and appetite has not yet returned. The following approaches can help maintain caloric intake during this period:

Warming food: Warm wet food to 35 to 37°C before offering. Do not microwave — heat unevenly and can create hot spots. Place the food container in warm water for 2 to 3 minutes.

Hand feeding: Offer small amounts of food from your finger or from a flat surface rather than a bowl. Some cats that refuse food from a bowl will eat from a hand, particularly if they associate the owner's presence with comfort.

Syringe feeding: If the cat is not eating voluntarily and has not eaten for more than 24 to 48 hours, syringe feeding is appropriate. Thin wet food with warm water or low-sodium broth to a smooth consistency. Use a 5 to 10 ml syringe without a needle. Administer small amounts (1 to 2 ml) slowly into the side of the mouth, allowing the cat to swallow between each administration. Do not force large volumes — aspiration risk is real in a weak cat.

Appetite stimulants: Mirtazapine (available as a transdermal gel applied to the inner ear flap) is the most commonly used appetite stimulant in cats and is safe to use alongside GS-441524. Capromorelin (Elura) is an alternative. Discuss with your vet which is most appropriate for your cat. Appetite stimulants are a bridge — they support intake while GS-441524 resolves the fever and systemic inflammation that are suppressing appetite.

Anti-nausea medication: Maropitant (Cerenia) reduces nausea and vomiting and can significantly improve food intake in cats with gastrointestinal involvement. It is safe alongside GS-441524. If your cat is vomiting or appears nauseous, discuss maropitant with your vet before the next meal. Cats receiving injectable GS-441524 may benefit particularly from anti-nausea support in the first week.

Foods to Avoid

The following are toxic to cats and must be avoided regardless of FIP status:

  • Onion, garlic, leeks, chives, and other allium vegetables — cause oxidative damage to red blood cells and haemolytic anaemia. This includes onion or garlic powder in any form.
  • Chocolate — contains theobromine, which is toxic to cats
  • Grapes and raisins — associated with acute kidney injury in cats
  • Xylitol — an artificial sweetener found in some human foods and supplements; causes hypoglycaemia and liver failure in cats
  • Caffeine — toxic to cats in any form
  • Alcohol — toxic to cats in any amount

Additionally, during active FIP treatment:

  • Raw meat, raw fish, and raw eggs — carry bacterial contamination risk (Salmonella, Campylobacter, Listeria) that a cat with a compromised immune system may not manage effectively. Use commercially prepared, heat-processed food during treatment.
  • Dairy products — most adult cats are lactose intolerant. Dairy causes gastrointestinal upset that compounds the nausea and appetite loss already present in FIP.
  • Highly processed human food — high sodium content, artificial additives, and preservatives are not appropriate for a cat in recovery.

Supplements: What Has Clinical Rationale

The following supplements have clinical rationale for use during FIP treatment. Always discuss with your vet before adding any supplement, particularly in cats with ongoing liver or kidney involvement.

Vitamin B12 (cobalamin): FIP reduces the cat's ability to maintain adequate B12 levels. B12 is essential for red blood cell production, neurological function, and appetite regulation. B12 supplementation is particularly relevant in cats with anaemia, neurological involvement, or persistent appetite loss. Injectable B12 (administered by your vet) has better bioavailability than oral supplementation in cats with gastrointestinal involvement.

SAMe (S-Adenosylmethionine): Supports hepatic glutathione production — the liver's primary antioxidant defence. Appropriate in cats with elevated liver enzymes during treatment. Available in feline-specific formulations.

Silybin (milk thistle extract): A hepatoprotective compound that supports liver cell membrane integrity. Commonly used alongside SAMe in cats with liver enzyme elevations. Available in feline-appropriate formulations.

Omega-3 fatty acids (EPA and DHA from marine sources): Anti-inflammatory effects, supports immune function and coat quality. Fish oil at feline-appropriate doses is safe and well-tolerated. Avoid cod liver oil — contains vitamins A and D at levels that can be toxic to cats at higher doses.

Mirtazapine: Primarily an appetite stimulant and anti-nausea agent rather than a nutritional supplement. Transdermal formulation (applied to the inner ear flap) is convenient and effective. Discuss with your vet.

Nutrition During the Observation Period

After completing the 84-day protocol, continue the high-protein wet food diet through the 12-week observation period. Most cats continue to gain weight and rebuild body condition during the observation period as the body completes its recovery. A cat that has fully restored body condition by day 84 can transition back to its normal diet, but maintaining good nutritional density supports immune function during the observation period.

Frequently Asked Questions

My cat has not eaten for 48 hours since starting GS-441524. What should I do?
Contact your vet about appetite stimulants (mirtazapine) and anti-nausea medication (maropitant). Begin syringe feeding with thinned wet food to maintain caloric intake. Do not stop or reduce the GS-441524 dose because the cat is not eating — the drug is suppressing the fever and inflammation that are causing the appetite loss, and appetite will return as treatment takes effect.

Can I feed my cat raw food during FIP treatment?
No. Raw meat carries bacterial contamination risk that a cat with a compromised immune system may not manage effectively. Use commercially prepared, heat-processed wet food during the 84-day protocol and the observation period.

My cat only wants to eat one specific food. Should I vary the diet?
During active treatment, the best food is the food the cat will eat. If the cat is eating one specific food consistently, continue offering it. Variety can be introduced gradually as appetite normalises and the cat is eating well. Do not force variety at the expense of caloric intake during the first weeks of treatment.

How much should my cat be eating during treatment?
A rough guide is 40 to 60 kcal per kg of body weight per day for a recovering cat — higher than the maintenance requirement for a healthy cat. A 4 kg cat needs approximately 160 to 240 kcal per day.
