Of the four forms of Feline Infectious Peritonitis, dry FIP is the one most likely to be missed. Wet FIP announces itself — a visibly swollen belly, laboured breathing, a cat that is clearly in distress. Dry FIP does not. What Is Dry FIP?
Dry FIP, also called non-effusive FIP, is a form of Feline Infectious Peritonitis in which the immune system partially contains the mutated feline coronavirus. Instead of the virus triggering the vasculitis and fluid leakage that characterises wet FIP, the immune response walls off infected cells and forms granulomas — small nodules of inflamed tissue — in the affected organs.

Symptoms of Dry FIP

Why Dry FIP Is So Often Missed
Dry FIP is frequently mistaken for other conditions because its symptoms overlap with so many of them:
- Lymphoma — particularly in older cats with weight loss and enlarged lymph nodes
- Toxoplasmosis — which can cause neurological and ocular signs similar to neurological and ocular FIP
- Inflammatory bowel disease — chronic vomiting, diarrhoea, and weight loss
- Idiopathic uveitis — eye inflammation without an identified cause
- Primary neurological disorders — when neurological signs appear without obvious systemic illness
The absence of abdominal fluid — the most visible sign of wet FIP — means that vets who are not specifically considering FIP may not run the diagnostic tests that would identify it. How Dry FIP Is Diagnosed
There is no single definitive test for dry FIP. Diagnosis requires combining multiple tools, and the combination that reaches the highest confidence is:
Albumin-to-Globulin (A:G) ratio — the most important blood marker for FIP. Below 0.6 is highly suggestive; below 0.4 is strongly indicative. The Hartmann et al. (2003, J Vet Intern Med) study established the A:G ratio as the most reliable single blood marker for FIP diagnosis. Ask your vet specifically for this test — it is not always included in a standard blood panel.
Full CBC and biochemistry profile — elevated globulins, low albumin, anaemia, elevated bilirubin, and elevated liver enzymes are all consistent with dry FIP. Low lymphocyte count is a particularly consistent finding.
PCR testing — detects feline coronavirus genetic material in tissue samples, cerebrospinal fluid, or aqueous humour (eye fluid). In neurological or ocular dry FIP, PCR on CSF or eye fluid can provide strong diagnostic evidence.
Cerebrospinal fluid (CSF) analysis — in cats with neurological signs, CSF analysis can detect the elevated protein and white cell count consistent with neurological FIP. This requires sedation or anaesthesia and is performed by a specialist.
Ultrasound — can reveal granulomas on the liver, kidneys, or lymph nodes, and enlarged mesenteric lymph nodes — findings that are highly suggestive of dry FIP in the right clinical context.
Histopathology — tissue biopsy of an affected organ or lymph node is the closest to a definitive diagnosis. It is invasive but provides the highest diagnostic certainty when other tests are inconclusive.

Treating Dry FIP With GS-441524
Dry FIP was once considered fatal. Today, GS-441524 antiviral treatment achieves a 92% success rate documented by UC Davis (Pedersen, 2019) across all forms of FIP, including dry FIP. The landmark Pedersen et al. 2019 trial (J Feline Med Surg) included cats with dry, neurological, and ocular FIP alongside wet FIP — all forms responded to GS-441524 at the appropriate dose.
