FIP in Kittens: Symptoms, Diagnosis, Treatment, and Prevention

Of all the cats that develop Feline Infectious Peritonitis, the majority are young. The Pedersen et al. 2019 UC Davis trial (J Feline Med Surg) — the landmark study that established GS-441524 as the standard of care for FIP — found that most FIP cases occur in cats under three years old, with kittens under one year representing a disproportionately large share of diagnoses.

This is not a coincidence. Kittens are the highest-risk group for FIP for reasons that are biological, environmental, and immunological. Understanding why — and knowing what to look for — is the most important thing a kitten owner or breeder can know.

Why Kittens Are at Highest Risk

Feline Infectious Peritonitis develops when the feline coronavirus (FCoV) — a common, usually harmless virus — mutates inside a cat’s body into a more aggressive form. In most cats, the immune system contains FCoV before this mutation can occur. In kittens, it often cannot.

A kitten’s immune system is still developing. Maternal antibodies, which provide some protection in the first weeks of life, wane by 6 to 8 weeks of age. The kitten’s own immune response is not fully mature until around 6 months. This window — from weaning to early adulthood — is when FCoV exposure is highest and immune control is weakest.

Compound this with the environments most kittens come from: shelters, catteries, and multi-cat breeding facilities where FCoV prevalence can reach 80–90%, according to research published in the Journal of Feline Medicine and Surgery (Addie & Jarrett, 1998). High viral load, immature immunity, and the stress of rehoming — a documented trigger for FCoV mutation — create the conditions where FIP is most likely to develop.

  • Genetic predisposition — Ragdolls, Bengals, Birmans, and Abyssinians are reported at higher rates in the literature
  • Co-infection with FeLV or FIV, which further suppresses immune function
  • Overcrowding and poor litter hygiene, which increase FCoV viral load
  • Stress from environmental changes, surgery, or illness

Symptoms of FIP in Kittens

FIP in kittens presents across the same four forms as in adult cats — wet, dry, ocular, and neurological — but often progresses faster because of the kitten’s less developed immune response. The general signs that appear across all forms are:

  • Persistent fever above 39.5°C that does not respond to antibiotics — the most consistent early sign
  • Loss of appetite — refusing food or eating significantly less than normal
  • Lethargy and weakness — a kitten that should be playful and curious becoming quiet and still
  • Weight loss or failure to gain weight — stunted growth in very young kittens
  • Rough or dull coat and reduced grooming

Beyond these general signs, form-specific symptoms appear:

Wet (Effusive) FIP

The most rapidly progressing form. Fluid accumulates in the abdomen or chest, sometimes within days of first symptoms appearing.

  • Visibly swollen, distended belly — often the first thing owners notice
  • Difficulty breathing or open-mouth breathing (chest fluid)
  • Pale gums or jaundice — yellowing of the gums or whites of the eyes
  • Rapid deterioration without treatment

Dry (Non-Effusive) FIP

More gradual, but equally serious. Granulomas form on internal organs without visible fluid accumulation.

  • Persistent low-grade fever and gradual weight loss over weeks
  • Enlarged lymph nodes or organs palpable on examination
  • Liver or kidney dysfunction on blood work
  • Jaundice in cases with liver involvement

Neurological FIP

Particularly distressing in kittens, as neurological signs can appear suddenly and progress quickly.

  • Wobbly, unsteady gait (ataxia)
  • Tremors or seizures
  • Head tilt or abnormal eye movements
  • Sudden behavioural changes — confusion, aggression, or disorientation

Ocular FIP

  • Cloudiness or colour changes in the iris
  • Uveitis — inflammation of the internal eye structures
  • Unequal pupil size
  • Vision impairment

Diagnosing FIP in Kittens

There is no single definitive test for FIP. Diagnosis requires combining multiple tools. In kittens, the clinical picture — age, environment, and symptom pattern — is itself an important part of the diagnostic picture.

Albumin-to-Globulin (A:G) ratio — the most important blood marker. Below 0.6 is highly suggestive of FIP; below 0.4 is strongly indicative. The Hartmann et al. (2003, J Vet Intern Med) study established the A:G ratio as the most reliable single blood marker for FIP diagnosis. Ask your vet specifically for this test — it is not always included in a standard blood panel.

Full CBC and biochemistry — elevated globulins, low albumin, anaemia, and elevated bilirubin are consistent FIP findings. Low lymphocyte count is a particularly consistent pattern.

Rivalta test — if abdominal or chest fluid is present, this simple bedside test has approximately 91% sensitivity for FIP, as documented in the Hartmann et al. study. In a kitten with a swollen belly, this is one of the first tests to perform.

PCR testing — detects feline coronavirus genetic material in fluid samples or tissue biopsies.

Ultrasound — confirms fluid accumulation in wet FIP and can reveal granulomas or enlarged lymph nodes in dry FIP.

Cerebrospinal fluid (CSF) analysis — in kittens with neurological signs, CSF analysis can detect the elevated protein and white cell count consistent with neurological FIP.

Treating FIP in Kittens With GS-441524

FIP in kittens used to be a death sentence. Today, GS-441524 antiviral treatment achieves a 92% success rate documented by UC Davis (Pedersen, 2019) when administered at the correct dose for the full 84-day protocol. Kittens respond well to GS-441524 — often showing returning appetite and reduced fever within the first 3 to 7 days of treatment.

The dose is calculated based on the kitten’s confirmed FIP form and exact body weight. Getting this right from the first injection is critical — under-dosing is the most common cause of treatment failure:

Supportive Care During Treatment

GS-441524 is the primary treatment. Supportive care helps the kitten maintain strength and comfort during the 84-day protocol:

  • Fluid therapy: Dehydrated kittens may require subcutaneous fluids, particularly in the early stages of treatment before appetite returns.
  • Corticosteroids: In some cases, particularly neurological and ocular FIP, your vet may prescribe a short course of corticosteroids to reduce acute inflammation at the start of treatment. This is a supportive measure — not a substitute for GS-441524.
  • Regular blood work: Every 30 days throughout the 84-day protocol to confirm treatment response. In growing kittens, weight checks every 2 weeks for dose adjustment.

Monitoring Progress

Preventing FIP in Kittens

FIP cannot be fully prevented, but the conditions that make FCoV mutation more likely can be managed — particularly in breeding facilities, shelters, and multi-cat households where kittens are most at risk.

Litter tray hygiene. FCoV spreads primarily through faeces. Scoop at least twice daily. Provide one tray per cat plus one extra. Wash trays weekly with hot water and cat-safe disinfectant. This is the single most impactful step in any multi-cat environment.

Reduce stress around rehoming. The period immediately after rehoming is one of the highest-risk windows for FCoV mutation. Minimise environmental changes, provide a quiet introduction space, and avoid additional stressors in the first 2 to 4 weeks after a kitten arrives in a new home.

Quarantine new kittens. Any new kitten entering a household with resident cats should be kept separate for 2 to 3 weeks before introduction. This reduces FCoV transmission risk in both directions.

FCoV testing in breeding facilities. Responsible breeders can test breeding cats for FCoV antibody status and manage FCoV-positive cats separately. Kittens born to FCoV-negative queens in FCoV-negative environments have significantly lower FIP risk.

Nutrition and immune support. High-quality, age-appropriate nutrition supports immune development in kittens. Avoid unnecessary stress, overcrowding, and early weaning, all of which compromise immune function.
