FIP vs Lymphoma in Cats: How to Tell the Difference
Two of the most serious diagnoses a cat can receive โ Feline Infectious Peritonitis and feline lymphoma โ can look almost identical in their early stages. Both cause fever, weight loss, lethargy, and appetite loss. Both can produce fluid accumulation in the abdomen or chest. Both affect young and middle-aged cats. And both are serious enough that a misdiagnosis in either direction has real consequences.
A cat with FIP that is treated for lymphoma receives chemotherapy that will not work and loses the window for GS-441524 treatment. A cat with lymphoma that is treated for FIP receives an antiviral that will not help and misses the opportunity for chemotherapy that might. Getting the diagnosis right is not a formality โ it is the difference between effective treatment and none at all.

Understanding FIP
Feline Infectious Peritonitis is caused by a mutation of the feline coronavirus (FCoV) that occurs inside an individual catโs body. The mutated virus infects macrophages and spreads systemically, triggering an immune-mediated inflammatory response that damages organs, blood vessels, and in some cats, the central nervous system or eyes.
FIP is most common in cats under two years old. It presents in four forms โ wet (effusive), dry (non-effusive), ocular, and neurological โ each with different dominant symptoms but sharing the same general signs: persistent fever above 39.5ยฐC that does not respond to antibiotics, appetite loss, lethargy, and gradual weight loss.

Understanding Feline Lymphoma
Feline lymphoma is a cancer of the lymphatic system โ the network of lymph nodes, vessels, and organs that forms part of the immune system. It is the most common feline cancer, accounting for approximately 30% of all feline tumours according to the Cornell Feline Health Center.
Unlike FIP, lymphoma is more common in middle-aged to older cats, though it can occur at any age. Cats positive for FeLV (feline leukaemia virus) are at significantly higher risk. Lymphoma is classified by its anatomical location, and each form has a different clinical presentation:
- Intestinal (alimentary) lymphoma: The most common form in cats. Causes chronic vomiting, diarrhoea, weight loss, and reduced appetite. Can be difficult to distinguish from inflammatory bowel disease and from dry FIP with intestinal involvement.
- Mediastinal lymphoma: A mass or fluid in the chest cavity causing breathing difficulty. Can look very similar to wet FIP with pleural effusion.
- Renal lymphoma: Kidney enlargement, increased thirst and urination, weight loss.
- Multicentric lymphoma: Affects multiple lymph nodes or organs simultaneously. Generalised lymph node enlargement is a key finding.
The general signs of lymphoma โ fever, lethargy, weight loss, reduced appetite โ overlap almost completely with FIP. The distinction lies in the details of the clinical picture, the blood work, and the diagnostic tests.
Key Clinical Differences
Age
Age is the single most useful initial differentiator. FIP is predominantly a disease of young cats โ the Pedersen et al. 2019 UC Davis trial (J Feline Med Surg) found that the majority of FIP cases occur in cats under three years old. Lymphoma is more common in cats over seven years old, though it can occur in younger cats, particularly those with FeLV infection.
A two-year-old cat with a swollen belly and fever is much more likely to have FIP than lymphoma. A ten-year-old cat with the same presentation is more likely to have lymphoma. Age does not confirm either diagnosis, but it significantly shifts the probability.
Blood Work
Blood work is where the most useful distinguishing information is found:

The A:G ratio is the most powerful single blood marker for FIP. The Hartmann et al. (2003, J Vet Intern Med) study established that an A:G ratio below 0.4 is strongly indicative of FIP. Lymphoma rarely produces this pattern of markedly elevated globulins with low albumin. Ask your vet specifically for the A:G ratio โ it is not always included in a standard blood panel.
Fluid Characteristics
When fluid is present, its characteristics are highly informative:

The Rivalta test โ a simple bedside test performed on effusion fluid โ is highly useful for distinguishing FIP from other causes of fluid accumulation. A positive Rivalta test in a young cat with the characteristic straw-coloured, viscous fluid is strongly suggestive of FIP. Lymphoma-related effusions typically produce a negative Rivalta result.
Imaging
Ultrasound findings differ between the two conditions:
- FIP: Free fluid with fibrin strands, enlarged mesenteric lymph nodes, organ granulomas, peritoneal thickening. The fluid is the dominant finding.
- Lymphoma: Discrete masses, diffuse intestinal wall thickening (particularly in alimentary lymphoma), well-defined lymph node enlargement. Masses are more characteristic of lymphoma than FIP.
Response to Steroids
Corticosteroids (prednisolone) are sometimes used diagnostically as well as therapeutically. Lymphoma โ particularly low-grade intestinal lymphoma โ often shows a temporary clinical improvement with steroid treatment. FIP rarely does. A cat that improves meaningfully on prednisolone alone is more likely to have lymphoma than FIP. A cat that continues to deteriorate despite steroids should be investigated urgently for FIP.
Definitive Diagnostic Tests
When the clinical picture and blood work are not conclusive, these tests provide the highest diagnostic certainty:
PCR for feline coronavirus. Detects FCoV genetic material in effusion fluid, CSF, or tissue. A positive PCR on effusion fluid in the right clinical context provides strong evidence for FIP. False negatives are possible, so a negative result does not rule out FIP.
Cytology of effusion fluid. Microscopic examination of cells in the fluid. FIP effusion typically shows a low cell count with predominantly neutrophils and macrophages. Lymphoma effusion may contain malignant lymphocytes โ a finding that confirms lymphoma.
Histopathology (biopsy). Tissue biopsy of an affected lymph node, intestinal segment, or organ provides the highest diagnostic certainty for both conditions. Histopathology can confirm FIP granulomas or lymphoma tumour cells definitively. It is invasive but removes diagnostic uncertainty when other tests are inconclusive.
Immunohistochemistry. Can identify FCoV antigen within macrophages in tissue samples โ the most specific test for FIP when tissue is available.

When to Suspect FIP Over Lymphoma
- Cat is under three years old
- Persistent fever above 39.5ยฐC not responding to antibiotics
- A:G ratio below 0.6 on blood work
- Straw-coloured, viscous effusion with positive Rivalta test
- No improvement with antibiotics or steroids
- Multi-cat household or shelter background (high FCoV exposure)
- Neurological or ocular signs alongside systemic illness
When to Suspect Lymphoma Over FIP
- Cat is over seven years old
- FeLV-positive status
- Discrete masses visible on imaging
- Diffuse intestinal wall thickening on ultrasound
- Malignant cells in effusion cytology
- Temporary improvement with corticosteroids
- A:G ratio normal or only mildly reduced
- Hypercalcaemia on blood work
What If the Diagnosis Is Still Unclear?
When the clinical picture overlaps and initial tests are inconclusive, the next step is tissue biopsy with histopathology. This is the most definitive way to distinguish FIP from lymphoma and should not be delayed if the cat is deteriorating.
In some cases, a therapeutic trial of GS-441524 may be considered when FIP is strongly suspected but cannot be confirmed โ particularly in a young cat with a low A:G ratio and characteristic effusion. A cat with FIP will typically show clinical improvement within 7 to 14 days of starting GS-441524 at the correct dose. A cat with lymphoma will not. This response โ or lack of it โ provides additional diagnostic information.
Any therapeutic trial should be conducted with veterinary oversight and clear criteria for reassessment. Do not continue GS-441524 indefinitely in a cat that is not responding โ reassess the diagnosis.
Treatment: Why the Distinction Matters
FIP: GS-441524 antiviral treatment achieves a 92% success rate documented by UC Davis (Pedersen, 2019) when administered at the correct dose for the full 84-day protocol. It has no meaningful effect on lymphoma.
Lymphoma: Treatment depends on the form and grade. Low-grade intestinal lymphoma is often managed with prednisolone and chlorambucil, with median survival times of 2 to 3 years in responding cats. High-grade lymphoma requires combination chemotherapy (COP or CHOP protocols). GS-441524 has no effect on lymphoma.
These treatments are not interchangeable. A cat with FIP that receives chemotherapy loses the window for effective antiviral treatment. A cat with lymphoma that receives GS-441524 loses the window for chemotherapy. Getting the diagnosis right is the treatment.

Frequently Asked Questions
Can a cat have both FIP and lymphoma at the same time?
