3 Dangerous Cat Viruses That Often Go Undetected Until It's Too Late

Cats are exceptionally good at hiding illness. It is an evolutionary trait — in the wild, a visibly sick animal is a vulnerable one. The result, for domestic cat owners, is that serious diseases can progress for days or weeks before the signs become obvious enough to prompt a vet visit. By then, the window for the most effective intervention has often narrowed significantly.

Three viral diseases in particular are responsible for a disproportionate share of late diagnoses in cats: Feline Infectious Peritonitis (FIP), feline coronavirus (FCoV), and feline calicivirus (FCV). All three share early signs that are easy to attribute to stress, a minor stomach upset, or a bad week. All three can become serious or fatal if the early signs are missed. And all three are manageable — or at least significantly more manageable — when caught early.

Virus 1: Feline Infectious Peritonitis (FIP)

What It Is

FIP is caused by a mutation of feline coronavirus (FCoV) that occurs inside an individual cat’s body. The mutated virus infects macrophages — the immune cells that are supposed to destroy pathogens — and uses them to spread systemically, triggering an immune-mediated inflammatory response that damages organs, blood vessels, and in some cats, the central nervous system and eyes.

Why It Goes Undetected

The early signs of FIP are among the most non-specific in feline medicine. The ABCD (Advisory Board on Cat Diseases) FIP guidelines specifically identify non-specific early symptoms as the primary reason FIP is underdiagnosed in its early stages. A cat with early FIP looks like a cat having a bad week:

  • Persistent fever above 39.5°C — the most consistent early sign, but easy to miss without a thermometer
  • Appetite loss — eating less, or eating but with less enthusiasm
  • Lethargy — quieter than usual, less playful, spending more time resting
  • Gradual weight loss over days to weeks

These signs appear before the form-specific signs — the swollen belly of wet FIP, the neurological changes of neurological FIP, the eye changes of ocular FIP — that make the diagnosis more obvious. By the time those signs appear, the disease has been progressing for some time.

The Four Forms and Their Specific Signs

Wet (effusive) FIP: Fluid accumulates in the abdomen or chest. The belly becomes visibly swollen. Breathing may become laboured if chest fluid is present. Dry (non-effusive) FIP: Granulomas form on internal organs without visible fluid. The cat continues to decline gradually — weight loss accelerates, fever persists, energy drops. Ocular FIP: Eye inflammation, cloudiness, colour changes in the iris, unequal pupil size. In some cats, eye changes are the first visible sign of FIP — appearing before systemic signs become obvious.

Why Early Action Changes Everything

FIP was considered universally fatal until 2019. The Pedersen et al. 2019 UC Davis trial (J Feline Med Surg) established GS-441524 antiviral treatment as the standard of care, achieving a 92% success rate when administered at the correct dose for the full 84-day protocol. The earlier treatment starts, the better the outcome — not because the drug works differently, but because there is less damage to reverse.

Virus 2: Feline Coronavirus (FCoV)

What It Is

Feline coronavirus is one of the most prevalent viruses in domestic cat populations. Research published in the Journal of Feline Medicine and Surgery (Addie & Jarrett, 1998) found FCoV prevalence of 25–40% in single-cat households and above 90% in catteries and shelters. Most cats are exposed to FCoV at some point in their lives — many before they leave the breeder or shelter.

FCoV spreads through the faecal-oral route: shared litter trays, grooming, and contaminated surfaces. The virus can survive on dry surfaces for up to seven weeks.

Why It Goes Undetected

In the vast majority of cats — approximately 98 to 99% of those infected — FCoV causes no symptoms at all, or at most mild, transient diarrhoea that resolves on its own. The immune system contains the virus in the intestinal tract, and the cat appears completely normal.

This is precisely why FCoV goes undetected: there is usually nothing to detect. The cat is infected, shedding the virus in its faeces, and potentially transmitting it to other cats — all without any visible signs of illness.

The danger is not FCoV itself. The danger is the mutation that occurs in approximately 1 to 2% of infected cats, transforming the intestinal coronavirus into the FIP virus. This mutation also produces no external signs — the cat may appear completely normal while the mutated virus is spreading systemically. What to Watch For

In a cat known to have been exposed to FCoV — through a shelter background, multi-cat household, or contact with an FCoV-positive cat — the following warrant prompt veterinary investigation:

  • Persistent fever above 39.5°C lasting more than 24 hours
  • Appetite loss that does not resolve within 24 to 48 hours
  • Sustained lethargy over multiple days
  • Any abdominal swelling or breathing difficulty

Virus 3: Feline Calicivirus (FCV)

What It Is

Feline calicivirus is one of the two most common causes of feline upper respiratory infection, responsible for approximately 40 to 50% of feline URI cases according to the ABCD FCV guidelines. It spreads through direct contact, shared bowls and litter trays, and aerosol transmission from sneezing. The virus can survive on surfaces for up to 28 days.

Why It Goes Undetected

FCV’s early signs — mild sneezing, slight nasal discharge, reduced appetite — are easy to attribute to a minor cold or environmental irritation. Cats hide oral pain instinctively, which means the mouth ulcers that are FCV’s most characteristic sign are often not discovered until they are large enough to prevent eating.

By the time a cat is refusing food entirely, the oral ulceration is typically well advanced. A cat that has been eating less for several days before refusing food entirely has been in pain for those several days — pain that was not visible because the cat was concealing it.

When FCV Becomes Dangerous

Most adult cats with intact immune function recover from FCV within 7 to 14 days with supportive care. But FCV becomes genuinely dangerous in three scenarios:

High-risk individual cats. Kittens, senior cats, and cats with compromised immune function — including those with FIV or FeLV — are at significantly higher risk of severe disease. In these cats, FCV can progress to pneumonia, severe dehydration, and rapid deterioration.

Chronic oral disease. Some cats develop chronic FCV-associated stomatitis — persistent, painful inflammation of the mouth that does not resolve with standard treatment. This is a significant welfare concern that requires long-term veterinary management and in some cases, full-mouth dental extraction.

Virulent systemic FCV (VS-FCV). A rare but serious variant that causes systemic illness rather than localised respiratory infection — high fever, facial and limb swelling, skin ulceration, jaundice, and haemorrhage. VS-FCV has a mortality rate of up to 67% even with aggressive treatment. Any cat showing these systemic signs alongside respiratory illness requires emergency veterinary care the same day.

What to Watch For

  • Sneezing or nasal discharge that persists beyond 3 to 4 days
  • Drooling or pawing at the mouth — signs of oral pain
  • Reduced appetite, particularly in a cat that normally eats well
  • Any facial or limb swelling, jaundice, or skin lesions — these require same-day emergency care
  • Refusing food for more than 24 hours — always warrants veterinary attention regardless of cause

Why These Three Viruses Are So Often Missed

The reason FIP, FCoV, and FCV are so frequently detected late is not that owners are inattentive. It is that all three share early signs — lethargy, reduced appetite, mild fever, gradual weight loss — that are genuinely non-specific. They look like stress. They look like a minor stomach upset. They look like a cat that is just having a quiet week.

The Cornell Feline Health Center notes that cats instinctively suppress visible signs of illness — which means that when a cat is showing even subtle signs of being unwell, the actual degree of discomfort is often greater than what is visible.

The practical implication: any of the following, sustained for more than 24 to 48 hours, warrants a veterinary visit rather than a wait-and-see approach:

  • Fever above 39.5°C
  • Appetite loss — eating significantly less than normal, or refusing food
  • Sustained lethargy over multiple days
  • Any weight loss that is visible or measurable
  • Sneezing, nasal discharge, or signs of oral discomfort
  • Any abdominal swelling, breathing difficulty, or eye changes

The Diagnostic Tests That Matter

When a cat presents with non-specific signs of illness, the following tests help distinguish between these three conditions and other causes:

For FCoV: FCoV antibody titre to confirm exposure. A positive result does not confirm FIP — it confirms FCoV exposure and indicates monitoring is warranted.

For FCV: Clinical diagnosis based on oral ulceration and respiratory signs. PCR testing of oral swabs can confirm FCV in ambiguous cases.

For all three: Full blood work including CBC, total protein, albumin, globulins, liver and kidney values, and bilirubin. Ultrasound if abdominal or chest involvement is suspected. FeLV and FIV testing to identify immunocompromised cats at higher risk of severe disease from any of these viruses.

Frequently Asked Questions

Can a cat have FIP and FCV at the same time?

My cat has been lethargic for three days but is still eating. Should I be concerned?

How do I know if my cat has been exposed to FCoV?

Is there a vaccine for FIP?
